For Our New Dental Patients in Edmonton
As a new patient at The Dentists at Northgate, there are a few things you can do beforehand to help make sure your initial appointment runs smoothly. Fill out and complete these patient forms prior to your appointment at our Edmonton office and let our office manager know that you have filled them out online.
